THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective date: October 21, 2026
Hand in Health Massage & Wellness LTD, doing business as Hand in Health Massage Therapy, is required by law to protect the privacy of your health information, give you this notice of our legal duties and privacy practices, notify you if a breach affects your unsecured health information, and follow the terms of the notice currently in effect. This notice applies to all care you receive from our massage therapists, chiropractor, acupuncturists and personal trainers.
How we use and share your health information
Treatment. We use your health information to provide and coordinate your care, and we share it among our providers and with other health care providers treating you. Example: your massage therapist may share notes with our chiropractor so your care plan works together, or we may send a treatment summary to the physician who referred you.
Payment. We use and share your health information to bill and get paid for your care. Example: we share information with the VA to be paid for care provided through the Community Care program, or we prepare a superbill you can send to your insurance.
Health care operations. We use your health information to run our practice, train staff, review quality and contact you when needed. Example: we use your visit history to send appointment reminders.
Business associates. We share information with companies that perform services for us, such as our scheduling and patient records system and our payment processor. They must sign an agreement to protect your information.
People involved in your care. We may share information with a family member, friend or caregiver involved in your care or payment, unless you tell us not to. For patients under 18, we share information with the parent or guardian who consented to treatment.
Other uses and disclosures allowed or required by law
We may share your information without your written permission when the law allows or requires it, including:

New York law gives extra protection to some sensitive information, such as HIV-related information. When state law is stricter than HIPAA, we follow state law.
